Before you start

The participant must have an active NDIS plan in CoordHub before you can add goals — goals are linked to a plan period.

Steps

Go to the participant's Goals tab

Open the participant profile and click Goals in the left sidebar.

Click Add Goal

Click Add Goal in the top-right corner.

Enter the goal details

Fill in:
  • Goal name — use the wording from the participant’s NDIS plan
  • Goal category — the NDIS domain this goal relates to (Daily Living, Community, Employment, Health & Wellbeing, Learning, Relationships, Home)
  • Linked plan — select the current active plan
  • Status — set to In Progress for a new goal
  • Notes (optional) — any context about what achieving this goal looks like, or functional sub-goals you’ve agreed with the participant

Save

Click Save. The goal appears on the participant’s Goals tab and is available to link against activities.

Well-written vs. vague goals

Use the participant’s own words where possible. The goal name should match (or closely paraphrase) the goal as written in the NDIS plan document. Vague goal (avoid):
Improve wellbeing
Well-written goal:
I want to improve my mental health and wellbeing by building daily routines and connecting with my community
The well-written version is specific enough for an NDIA planner to assess whether funded supports have contributed to the outcome.

Updating goal status

When a goal is achieved or discontinued, open the goal record, change the status, and add a note describing the outcome. The previous status history is retained in the audit log. For achieved goals: describe what the participant can now do that they couldn’t before. This language is directly useful for progress reports.
